in-network

We are currently contracted with Blue Cross Blue Shield of Vermont and Medicare (more coming soon!). Please consult your insurance plan for information about coverage, co-pays and deductibles.

You should confirm your physical therapy coverage (including whether or not you need a referral) with your insurance carrier prior to making an appointment with us.

If you have a Medicare Advantage plan you should look to see if you have out-of-network benefits to see if your care with us will be covered

Please note that Medicare typically covers 80% of the cost of PT services. Some people have supplemental plans that cover the other 20%. We may or may not be in-network with your supplemental plan. You should confirm your insurance coverage before booking an initial evaluation.

Out-of-network

Aren’t covered by one of our insurance partners? You still have options!

• Check with your insurance plan about out-of-network benefits for physical therapy. Many plans offer partial or full coverage for out-of-network care.

• Use your HSA/FSA or other medical spending account. You can pay for out-of-network PT with money in these accounts.

Good Faith Estimate

If you do not have health insurance or plan to self-pay for our services, we will provide you with a “Good Faith Estimate” of cost when you schedule at least 3 days in advance. If you would like to request a Good Faith Estimate before scheduling, please call our office at (802) 222-0201 or email kerry@gmcf.life.
